Base Reports on Facts: All submissions should be rooted in factual information and, when possible, supported by available documentation or evidence.

Accuracy Matters: Knowingly submitting false or misleading information is a violation of our standards and may result in disciplinary action in accordance with company policies.

Your trust is paramount. We strictly prohibit retaliation against any employee who reports concerns in good faith.Your identity and the details are handled with the highest level of discretion.
🛑 Immediate Danger?
If someone is in immediate physical danger, or a security breach is actively occurring, do not use these forms. Contact your supervisor, HR, or the Facility Security Officer (FSO) immediately, or dial 911.
🌐 Choosing Your Reporting Method: QR Code vs. Web Link
You can submit your report via the web links below or by scanning the QR codes. Both forms are anonymous, but they handle network data differently:

📲 Scan the QR Code (Most Private): For maximum anonymity, scan the QR code using a personal mobile device on cellular data (not company Wi-Fi). This completely isolates your connection from company network logs.

💻 Click the Web Link: If you click the link from a company computer or while on company Wi-Fi, standard network logs will record where and when the link was accessed, which could theoretically be traced back to your workstation.
Personnel Security & Clearance Compliance Form
Use this secure form to fulfill mandatory reporting obligations or to share a security concern regarding another individual. Timely disclosure of specific life events and behavioral changes is required for all personnel maintaining a security clearance to ensure continuous eligibility and protect our mission.

Dual-Purpose System: This form handles both Self-Reporting (mandatory personal disclosures for cleared personnel) and Third-Party Reporting (reporting a concern about another individual).

Categories Covered: Use this to report personal changes (name, marital status, etc.), legal/criminal incidents, financial concerns, substance or behavioral concerns, and foreign influence triggers (such as foreign contacts or financial assets).

Foreign Travel Compliance for Cleared Personnel
All personnel holding a security clearance must complete the standard three-step reporting process for any planned personal or professional travel outside of the United States. This workflow ensures compliance with federal defense reporting requirements and helps safeguard your security eligibility.
Please complete the following actions in order:

Pre-Travel Questionnaire: Submit your itinerary, travel dates, and contact information at least 1 week prior to departure.

High-Risk Destination Exception: If your travel involves high-risk countries—including China, Russia, Iran, North Korea, Cuba, Syria, Yemen, Ukraine, Pakistan, Iraq, Haiti, and Afghanistan—your Pre-Travel Questionnaire must be submitted at least 30 days prior to departure. An official DCSA Counterintelligence Briefing may be required before your departure.

Pre-Travel Security Briefing: Review the mandatory threat briefing and situational awareness guidelines before you leave.

Post-Travel Debriefing: Complete this follow-up questionnaire within 5 business days of your return to report itinerary changes and any unusual contacts or security incidents.
